VA Benefits for Seniors in Illinois
Illinois is home to approximately 670,000 veterans. This guide covers every VA and state benefit available to senior veterans in Illinois — Aid & Attendance, VA medical centers, state property tax exemptions, veterans homes, and Medicaid coordination.
VA Aid & Attendance — the most overlooked senior benefit
The VA Pension with Aid & Attendance is a tax-free monthly benefit for wartime veterans (and surviving spouses) who need help with activities of daily living — whether they live at home, in assisted living, or in a nursing home. Fewer than 40% of eligible veterans actually receive it. It does not require a service-connected disability.

Who qualifies for Aid & Attendance?
- Veteran served at least 90 days on active duty with at least one day during a wartime period (WWII, Korea, Vietnam, Persian Gulf)
- Honorably or generally discharged
- Need help with activities of daily living (bathing, dressing, eating, toileting, ambulating)
- Income falls below the Maximum Annual Pension Rate (MAPR) — net income after medical/care expenses often qualifies people who initially seem too high
- Assets generally under $155,356 (2025 threshold, indexed annually); primary residence and vehicle excluded
3-year look-back rule (since 2018): Asset transfers in the 36 months before applying can trigger a penalty period. Consult a VA-accredited attorney before gifting assets or creating trusts.
VA Medical Centers in Illinois
These are the primary VA medical centers serving Illinois veterans. Each VAMC also operates a network of Community-Based Outpatient Clinics (CBOCs) in smaller cities — call the main number to find the nearest clinic.
For Community Care (approved outside-VA care), call the VA Community Care Program: 866-606-8198. Veterans who live more than 40 miles from a VA facility or face long wait times are often eligible for community care with a VA-approved local provider.
Illinois-specific veteran benefits
Beyond federal VA benefits, Illinois offers its own programs. Contact the Illinois Department of Veterans Affairs at 800-437-9824 or visit illinois.gov/veterans to apply.
Illinois Veterans Care (IVC): income-based health care for Illinois veterans not eligible for VA health care — 300 clinics statewide
Property tax relief: veterans with service-connected disability exemption on primary residence; 100% disabled veterans receive full homestead exemption
Illinois Veterans Home network: state-operated homes in Anna, LaSalle, Manteno, Quincy — priority for 60+ days active duty veterans
Illinois Veteran Grant (IVG): tuition waiver at Illinois public universities for Illinois veterans and dependents
Veterans Service Organizations: American Legion, VFW, DAV offices throughout Illinois assist with benefit claims
VA benefits + Illinois Medicaid: how they work together
Illinois Medicaid (HFS) covers nursing home care for income and asset-eligible veterans. The Illinois Medicare-Medicaid Alignment Initiative (MMAI) integrates benefits for dual-eligible seniors in select counties. VA Aid & Attendance income is excluded from Illinois Medicaid income calculations.
Key rule: A&A income is excluded
The VA Pension / Aid & Attendance benefit is generally not counted as income for Medicaid long-term care eligibility in most states — but the rules are complex and program-specific. Always verify with the Illinois Medicaid office or an elder law attorney before applying.
Dual-eligible planning
Veterans who qualify for both VA and Medicaid should work with a VA-accredited claims agent AND an elder law attorney — each program has its own asset and income rules. Proper coordination can significantly reduce out-of-pocket care costs.
Illinois State Veterans Homes
Illinois operates state-funded veterans homes offering skilled nursing, domiciliary, and memory care for eligible veterans. The VA pays a daily per diem to the facility for VA-eligible residents, significantly reducing costs. Medicaid is also accepted at most state veterans homes.

How to apply for VA benefits in Illinois
- 1
Gather discharge documents
You need your DD-214 (Certificate of Release or Discharge from Active Duty). If you can't find it, request it for free at archives.gov/veterans/military-service-records.
- 2
Contact a free Veterans Service Organization (VSO)
VSOs like the American Legion, DAV, VFW, and AMVETS provide free claims assistance. The Illinois Department of Veterans Affairs (800-437-9824) can refer you to an accredited VSO near you. VSO assistance is free — never pay a third party to file a VA claim.
- 3
File your claim
For Aid & Attendance: submit VA Form 21P-527EZ (Application for Pension) and VA Form 21-2680 (Examination for Housebound Status or Permanent Need for Regular Aid & Attendance). Attach a physician's statement documenting your care needs.
- 4
Apply for state-specific benefits
Property tax exemptions and state-specific programs require a separate application with the Illinois Department of Veterans Affairs. Some benefits are processed through county offices — your VSO can guide you to the right office.
- 5
Coordinate with Medicaid if applicable
If you're applying for both VA benefits and Illinois Medicaid long-term care, consult an elder law attorney. An attorney can help you structure your assets and applications to maximize benefits from both programs without disqualifying yourself from either.

These rates are indexed annually and are federal amounts — they apply uniformly in all 50 states including Illinois.
Does Illinois Medicaid count VA benefits as income?▾
Illinois Medicaid (HFS) covers nursing home care for income and asset-eligible veterans. The Illinois Medicare-Medicaid Alignment Initiative (MMAI) integrates benefits for dual-eligible seniors in select counties. VA Aid & Attendance income is excluded from Illinois Medicaid income calculations.
What property tax benefits does Illinois offer veterans?▾
Veterans with service-connected disability: standard exemption. 100% disabled: full homestead exemption on primary residence. Contact the Illinois Department of Veterans Affairs at 800-437-9824 or visit illinois.gov/veterans to apply and confirm current exemption amounts, as county assessor offices process the applications and amounts can vary by locality.
How do Illinois State Veterans Homes work?▾
Illinois State Veterans Homes are residential care facilities operated by the state for eligible veterans, surviving spouses, and in some cases gold star parents. They offer skilled nursing care, domiciliary (assisted living) care, and sometimes memory care. Costs are subsidized: eligible veterans pay significantly less than comparable private facilities. The VA provides a per diem payment directly to state veterans homes for VA-eligible residents, further reducing out-of-pocket cost. Medicaid coverage is also accepted at state veterans homes in most cases. Contact 800-437-9824 to apply.
Can a veteran use both VA benefits and Medicaid at the same time in Illinois?▾
Yes — dual eligibility is common. Illinois Medicaid (HFS) covers nursing home care for income and asset-eligible veterans. The Illinois Medicare-Medicaid Alignment Initiative (MMAI) integrates benefits for dual-eligible seniors in select counties. VA Aid & Attendance income is excluded from Illinois Medicaid income calculations. Veterans who qualify for both VA benefits and Medicaid should work with an elder law attorney or VA-accredited claims agent to coordinate the two programs so neither benefit disqualifies the other. The VA pension and Aid & Attendance benefit are structured specifically so they can supplement Medicaid-covered care.
